Scalded skin syndrome associated with staphylococcemia in a neonate: a case report

Abstract

Staphylococcal scalded skin syndrome is a pathology that affects the epidermis at the level of the stratum granulosus, which is triggered by the action of toxins produced by certain strains of Staphylococcus aureus (S. Aureus)Objective: to describe a case of scalded skin syndrome, highlighting the clinical presentation and its evolution.  Clinical case: 38 weeks newborn, with a clinical consisting of generalized cutaneous erythema and desquamation in the perioral region, neck and armpits edema, edema of the lower limbs and quantified fever.  She was hospitalized with diagnosis of late neonatal sepsis and scalded skin syndrome, under antibiotic coverage with oxacillin + amikacin. The report of blood cultures shows growth of S. Aureus methicillin resistant, management is left with vancomycin for 14 days after negative control blood cultures, finally discharged  at 19 days of hospitalization, without complications.  Conclusions: despite the low incidence of scalded skin syndrome, neonates are a group particularly susceptible to infections by toxins of S. Aureus, due to their decreased renal function; which makes their elimination less efficient, as well as their immunological immaturity.
