Kounis syndrome secondary to apidic accident: A case report

Abstract

Kounis syndrome is a cardiac condition characterized by the occurrence of coronary spasm produced by the massive release of inflammatory mediators such as histamine, which cause dysfunction or spasm of the epicardial coronary vessels, potentially leading to myocardial ischemia. There are three variants: type I or coronary vasospastic variant, in which coronary spasm occurs without ST-segment elevation; type II or infarction variant, with ST-elevation due to pre-existing coronary atherosclerosis; and type III, associated with stent thrombosis. The diagnosis is based on the temporal coexistence of allergic or anaphylactic symptoms together with acute ischemic cardiac symptoms, usually accompanied by suggestive electrocardiographic changes. Treatment consists of reversing the allergic reaction with epinephrine, antihistamines, and corticosteroids, in addition to anti-ischemic measures, such as nitroglycerin and antiplatelet agents. Then, we discuss the case of a patient who presents a hymenoptera sting accident and develops complications such as anaphylaxis, rhabdomyolysis, and Kounis syndrome.
