Orthodontic Management of an Immature Tooth Following Apexification Due to Dentoalveolar Trauma. A 5-Year Follow-up Case Report

Abstract

This case report describes the orthodontic management of an immature permanent tooth following apexification due to dentoalveolar trauma in a 9-year-old female patient.
The trauma involved a complicated crown fracture and intrusive luxation, resulting in pulp necrosis in tooth 11 and subsequent loss of tooth 21. A regenerative endodontic procedure was attempted but was unsuccessful due to the inability to induce apical bleeding; therefore, apexification with Biodentine™ was performed to ensure apical closure. The orthodontic treatment, planned in phases, focused on moving the adjacent teeth into the space left by the missing tooth 21, applying light forces to minimize the risk of root resorption. After five years of follow-up, successful root development and stability of the periapical tissues were observed, allowing for esthetic rehabilitation with composite resin veneers.
This long-term follow-up confirmed the stability of the treatment and the feasibility of a conservative approach in a growing patient. This case highlights the importance of a multidisciplinary approach —combining endodontics, orthodontics, and prosthetic rehabilitation— to achieve favorable clinical outcomes. Careful planning and the use of conservative orthodontic forces were key to preventing long-term complications and ensuring a satisfactory esthetic result.
