Outcomes of Primary and Secondary Decompressive Craniectomy in Patients with Severe Traumatic Brain Injury at Clínica de la Universidad de La Sabana

Abstract

Objectives: The purpose of this study was to determine the outcome in the discharge and follow-up to one year of patients with severe cranioencephalic trauma who underwent primary and secondary decompressive craniectomy at the Clínica de la Universidad de La Sabana, over a period of five years.

Patients and methods: A series of retrospective cases of patients with severe head trauma who underwent decompressive craniectomy between 2008 and 2013 were performed. The primary outcomes were survival and functional status as measured by the Glasgow outcome stage at the time of hospital discharge and at the year of follow-up. Secondary outcomes included latency time for craniectomy, intra- and postoperative complications, days of hospitalization and stay in the intensive care unit, ventilation time, Cranioplasty results and cause of death.

Results: Thirty-five patients with severe head trauma underwent decompressive craniectomy during the study period, 29 primary and 6 secondary patients, with a median latency of 5 hours and 57 hours, respectively. A survival rate of 51.4% of patients was observed, of whom 39% had satisfactory functional recovery on the Glasgow outcome scale at the time of discharge and at one year.

Conclusions: In this group of patients who underwent primary or secondary decompressive craniectomy, along with interdisciplinary management and early rehabilitation, there were favorable functional outcomes in long-term follow-up.
